Conceiving a child is a shared journey. When couples experience difficulty getting pregnant, factors related to male fertility contribute in up to 50% of cases. Male fertility depends on the healthy production, transportation, and functional quality of sperm.
A reproductive urology evaluation helps identify the underlying causes of male factor infertility—many of which are highly treatable with lifestyle changes, medical therapy, or minor procedures.
Common Causes of Male Infertility
Male infertility can stem from hormonal imbalances, structural
blockages, or genetic factors:
- Varicocele: Swollen, enlarged veins in the scrotum (similar to varicose veins in the legs). Varicoceles increase scrotal temperature, which can impair sperm production and movement.
- Structural Blockages (Obstruction): Scarring, prior infections, or congenital absence of the vas deferens (the tube that carries sperm) can prevent sperm from mixing into the ejaculate.
- Hormonal Imbalances: Deficiencies in pituitary or testicular hormones (such as Low Testosterone, LH, or FSH) that signal the body to produce sperm.
- Ejaculatory Disorders: Conditions such as retrograde ejaculation, where semen travels backward into the bladder during climax rather than out through the penis.
- Lifestyle & Environmental Factors: High heat exposure (hot tubs, saunas), smoking, heavy alcohol use, anabolic steroids, severe stress, and exposure to certain environmental toxins.
Diagnostic Evaluation:
Identifying the cause of fertility challenges begins with a thorough and respectful evaluation:
- Comprehensive Semen Analysis: The fundamental test assessing sperm count (concentration), motility (movement), morphology (shape), and total volume.
- Hormonal Blood Testing: Evaluating levels of Testosterone, Follicle-Stimulating Hormone (FSH), Luteinizing Hormone (LH), and Prolactin to evaluate sperm signals.
- Scrotal Ultrasound: A non-invasive imaging test used to evaluate the testicles and check for varicoceles, cysts, or anatomical abnormalities.
- Genetic Testing: Specialized blood tests (such as karyotyping or Y-chromosome microdeletion analysis) for patients with very low sperm counts or a complete absence of sperm (azoospermia).
Treatment Options
Treatment is customized based on the specific cause identified during your diagnostic workup:
- Microsurgical Varicocelectomy – A minimally invasive procedure performed using an operating microscope to tie off enlarged veins in the scrotum. Redirecting blood flow to normal veins lowers scrotal temperature, which can significantly improve sperm count, motility, and DNA quality.
- Hormonal Therapies – For men with low hormone levels, medical therapies can stimulate endogenous sperm production:
- Clomiphene Citrate / Enclomiphene: Medications that encourage the pituitary gland to produce more LH and FSH, signaling the testes to produce more natural testosterone and sperm.
- Human Chorionic Gonadotropin (hCG): Injections that directly mimic LH to stimulate sperm production while preserving natural fertility (unlike exogenous testosterone therapy, which suppresses sperm production).
- Surgical Sperm Retrieval – For men with blockages or severe sperm production issues, sperm can often be retrieved directly from the testicle or epididymis for use in Assisted Reproductive Technology (such as IVF with ICSI):
- TESE (Testicular Sperm Extraction): Retrieving small samples of testicular tissue to extract viable sperm.
- Micro-TESE: An advanced microsurgical technique used to locate microscopic pockets of sperm production in men with severe non-obstructive azoospermia.
- MESA / PESA: Minimally invasive extraction of sperm from the epididymis.
- Vasectomy Reversal (Vasovasostomy / Vasoepididymostomy)
A specialized microsurgical procedure that reconnects the vas deferens tubes to restore natural sperm flow for men who previously had a vasectomy.

In addition to medical and surgical treatments, small lifestyle adjustments can make a meaningful impact on sperm health:
- Nutritional Optimization: Incorporating antioxidants (such as Vitamins C and E, Zinc, CoQ10, and L-carnitine) to protect sperm from oxidative stress.
- Temperature Control: Avoiding frequent hot tub use, saunas, and prolonged tight clothing.
- Medication Review: Evaluating current prescription medications and immediately stopping any anabolic steroids or unprescribed testosterone supplements, which shut down natural sperm production.
